The Future of Healthy Ageing: Person-Centric Community Care

Technology has created a unique opportunity to place seniors at the centre of their health, enable ageing in place, and shift toward preventative models of care. To meet the challenges of a “super- aged” society, we must achieve alignment of health system-level strategies with digital infrastructure and tools to improve the lives of seniors.

A New Era of Personalized Care for Seniors:

Advances in genomics, precision medicine and digital therapeutics have the potential to reshape how we detect and manage chronic disease, enabling prevention-focused care models that can be tailored to each senior.

Empowering the Community Health Enterprise:

Establishing integrated and secure digital health infrastructures makes high-quality care accessible and affordable outside of an acute care setting. Analytic tools will meaningfully support clinicians and enable health systems to achieve continuous improvement and innovation.

Helping Seniors to Maintain Independence:

Wearables and sensors will allow ageing in place, enhance the quality of life for seniors, and provide better coordination and support for caregivers and families.

We are honoured to have among us Singapore Ministry of Health officials, medical professionals and scientists, international entrepreneurs and investors, technological companies at the forefront of chronic disease preventive and early detection aged care, and related Singapore government agencies.
